COVID-19 Vaccination Certificate for Urgent Travel

A COVID-19 vaccination certificate allows you to prove that you have had a full course of the vaccine.

Do not contact our GP surgery about your COVID-19 vaccination status. Your GP surgery cannot provide you with a letter or proof of your immunisation.

How to request a certificate if required for urgent travel

Request a certificate only if:

  • you have had both doses of the COVID-19 vaccine (you should wait 5 days after your second dose before requesting it)
  • you urgently need to travel to a country which requires vaccine certification
  • you are not able to either quarantine or provide tests to meet country entry requirements

You can now either request a Digital COVID Pass (recommended) or a paper certificate.

Digital: England - You can access your Covid pass by cliclking either  NHS COVID pass England or NHS COVID pass Wales for international travel online and download or print the Pass as a PDF document. To access the service, you'll need to register for an NHS login if you do not have one already. To register for an NHS login you'll need to upload a photo of your ID (passport, full UKL driving licence).

If you do not have photographic ID available to get your NHS COVID Pass you'll need to request a letter via phone (as below).
